What Can Your Tongue Tell You? A Traditional Chinese Medicine Perspective

Have you ever been asked to stick out your tongue during an acupuncture appointment? It may seem unusual, but tongue diagnosis is one of the oldest and most valuable assessment tools in Traditional Chinese Medicine (TCM).

Your tongue provides a snapshot of your body’s internal patterns. By observing its colour, shape, coating and moisture, Chinese Medicine practitioners gather information that is considered alongside your symptoms, medical history, pulse and lifestyle. Tongue diagnosis is never used on its own to diagnose a medical condition, but it can help guide an individualised treatment plan.

Common Tongue Findings in Chinese Medicine

💜 Purple Tongue

A purple or bluish tongue may suggest Blood Stasis, a pattern associated with reduced circulation or stagnation. People with this pattern may experience chronic pain, old injuries, menstrual pain, or poor circulation. Treatment aims to improve the smooth flow of Qi and Blood.

❤️ Red Tongue

A red tongue often indicates a pattern of Heat, which may be associated with inflammation or excess activity within the body. Depending on the person’s presentation, this could accompany symptoms such as irritability, night sweats, mouth ulcers, or feeling unusually warm.

🤍 Pale Tongue

A pale tongue may suggest Qi Deficiency or Blood Deficiency. This pattern is commonly seen in people who experience fatigue, dizziness, poor concentration, or who feel cold easily. Treatment focuses on supporting the body’s energy and nourishing Blood.

💦 Swollen Tongue

A swollen tongue, sometimes with teeth marks around the edges, may indicate Dampness or a weakness in digestion according to TCM theory. People may experience bloating, heaviness, fluid retention or low energy.

🏜️ Cracked Tongue

Cracks in the tongue can have several meanings depending on their location and depth. In TCM they are often associated with Yin Deficiency, which may present with dryness, night sweats, thirst or symptoms that worsen later in the day.

🤍 Thick Tongue Coating

A thick white or yellow coating may suggest an accumulation of Dampness or Phlegm, often related to digestive imbalance or the body’s response to illness. The colour and thickness of the coating provide additional clues for the practitioner.
